MSHSAA Preparticipation Physical Forms

Families of students participating in MSHSAA activities are encouraged to consider scheduling sports physicals during the summer months of June, July, or August.

Physicals are now valid for two full years of participation in MSHSAA activities. Completing a physical during the summer can help prevent the possibility of it expiring during the school year, which could result in missed participation time during a sports season or activity.
